The Flu Shot Clinic is scheduled for Tuesday, November 1st from 11:30 am to 1:30 pm.

The cost to all Bayard employees is $10.00. If you pay by check, please make the check payable
to Bayard as they are covering the additional $10.00 fee for everyone as well as the hourly nurse
fees.

Please see me for consent forms (which need to be handed in prior to your flu shot) and an
information sheet.
